Hope you are doing well and thank you for your cooperation with the following changes: 

 

*UPDATED* COVID Regulations 

 

1.     Fill out the FAFS surveys IN REAL TIME for EACH BUILDING (ie. Plant Science, Agriculture Building (includes the greenhouse), The Point, CDC, etc.)  

• When you enter the building, fill out the survey AT THAT TIME. 
• When you exit the building, fill out the survey AT THAT TIME. 

 

NOTE: If you move between Plant Science and the Agriculture Building, these are two SEPARATE buildings and you need to fill out the survey as if you are physically entering and exiting two separate buildings.

 

Tip: Use the QR codes at the entrances/exits to make this faster.    

 

The time period between your entering survey response and exiting survey response are tracked – it needs to be accurate to show how many people from your lab are physically present at one time.  

 

2.     If someone in your lab won’t be using their approved slot or wants to switch, you can BUT you need to let your PI/Lab Supervisor know (and they will alert the Department Head). You still need to fill out the FAFS surveys as if it was your slot.
