As the Winter 2026 term approaches, I would like to kindly remind all students that they are required to submit a ‘Self-Declaration Form for Brief or Temporary Absence’, for every absence.

A link to this form can be found in the ‘Respiratory Therapy Program Information’  course on UM Learn, under ‘BRT/CoRS Policies’.

It is important that you use this Microsoft Form to notify the department of your absence(s) as it will automatically distribute your communication directly to those who need to be informed, as well as help with tracking/safety purposes.

Please do not send emails to your instructor or RT Admin notifying of your absence as you will just be redirected to submit the MS Form.
