Hello Dent 1 Students,
As you are coming to the end of the year & the start of Clearance, here are a few reminders to help keep you focused:
- Clearance Sign-Up sheets will be locked end of day
May 8
- Double check all your appointment times are in the correct order:
- Appointments are not over lapping
- Please enter your first & last name
- If you have a student balance on your Student Axium account, please pay at Reception
BEFORE going to see Michelle for Account Balances
- Account Balance with Michelle is your
LAST appointment after getting ALL other signatures
How to find your KIT List in your Student Axium account:
- Open Axium
- Type students name (Student chart is with the numbers only– do not use the chart with a Letter)
- Click on
Attachments
- Scroll down to
Paper Chart Info
- Scroll over & select tab Agreement/Contract (there are 6 tabs & this is the last tab – you can’t see it if your screen isn’t expanded – you have to click on the arrow & scroll
to the end)

Hello Dent 1 Students,
Read all information – then read it again.
If you have questions, please come & ask before Clearance starts. There are things you need to clear up before clinic ends.

Things to remember for CLEARANCE:
- All CLEARANCE information can be found on Dent 1 Clearance TEAM!
- Students must sign-up for all appointments by
May 8. The file will be locked after this date.
- Students must show up at their
respective appointment times as such, other accommodations cannot be made.
- It is
imperative that you have all forms & reports completed & approved by your instructor
PRIOR to your clearance appointments.
- Pay attention to the days & times of your sign-up appointments (some clearance needs to be done in a specific order & your appointments should
not overlap each other).
- Please enter your first & last name on each sign-up sheet.
- Michelle Oshanyk for your Account Balance, must be the last person you meet with by
May 20.
