Hello All,

 

We understand there have been some questions about preparing your disposition lists for clearance. Please follow instructions below.

 

1 - Pull your current patient list on axiUm.

2 - Compare your axiUm patient list with your disposition sheet on Microsoft Teams.

If your Microsoft Teams disposition sheet is missing any patients appearing on your axiUm patient list, you must add those patients to your disposition sheet. (No axiUm disposition forms accepted after March 13th, 2026.)

 

 

To pull your current axiUm list follow the steps below:

 

Go to: Personal Planner Module on axiUm

Select: Assigned Pts tab

Select: Date From and Date To (set each field to the current date)

Select:  to populate your current list

A reminder if you have any patients assigned to you after you have filled out your disposition sheet, you will need to update it to match your current axiUm patient list up until April 20, 2026.
