Students must sign-up for all appointments by May 1, 2026, the file will be locked after this date.
Students must show up at their respective appointment times as such, other accommodations cannot be made.
It is imperative that you have all forms & reports completed & approved by your instructor prior to your clearance appointments.
Pay attention to the days & times of your sign-up appointments (some clearance needs to be made in a specific order & your appointments should not overlap each other).
Please enter your first & last name on each sign-up sheet.
