and package. Please select new dates for your appointment.
Read all information – then read it again. If you have questions, please come & ask before Clearance starts. There are things you need to clear up before the clinic ends.

Things to remember for CLEARANCE:
- All CLEARANCE information can be found on D4 Clearance TEAM!
- Students must sign up for all appointments by April 24, 2016 . The file will be locked after this date.
- Students must show up at their respective appointment times as such; other accommodations cannot be made.
- It is imperative that you have all forms & reports completed & approved by your instructor prior to your clearance of appointments.
- Pay attention to the days & times of your sign-up appointments (some clearance needs to be made in a specific order & your appointments should not overlap with each other).
- Please enter your first & last name on each sign-up sheet.
- Michelle Oshanyk for your Account Balance, must be the last person you meet by May 8, 2026, after you have received all signatures on the Clearance Checklist.
