Dear Students;

In order to prepare your hospital computer account logins for this coming fall academic year, you are required to complete the Account Management Authentication Q & A and the WRHA Clinical Information Systems Access Agreement forms that are attached. 

You will be provided with read/viewing access to assist you with your clinical skills sessions in Med 2 starting this coming Fall 2026.  You will be sent a notification email when this has been completed with Manitoba eHealth.

To fill out the form electronically, please open the form in Adobe Acrobat then choose Tools then Fill & Sign.

Both forms are to be returned to this email prior to January 30, 2026 @ 4 pm.

Please do not email or fax your forms to Shared Health. The UGME office will do so for you.
