Thank you for all of the responses for the Audio Metric Test. ESHO has asked that we provide them with some more information.

The same criteria still apply (exposure to sound 80 DPI or over for the hearing tests that EHSO provides)

Please Provide the following additional information:

Location (Building/Room):

Equipment/ Tool (over 80 decibel):

*Please list all of the equipment that you work with that would be over 80 decibels*

This new information will help EHSO determine areas/ equipment that need to be check for noise levels.
