[Dental-hygiene-year2] Complete the Computer Usage Agreement

Hello DH 2 Students,

Response required by Friday October 24, 2025.

As you will begin seeing patients in January 2026, you will have access to their records in AxiUm. You are required to sign the Computer Usage Agreement form to be granted remote access.
