Dental Hygiene and Dental Students,
Updates as follows:
1. For insurance pre-authorizations (CDCP, Blue Cross, NIHB etc.) you must indicate if a planned crown/denture is "Initial Placement". If not the initial placement, the PCCs need to know approximately when the initial was placed in order to submit the pre-authorization. If this detail is not included in the pre-authorization request it will be denied and returned requesting this information. Add these details to EHR notes when treatment planning is most helpful. You can also include the details in your email request for pre-authorization submission. 2. HC FUND Form * If you want to request the Exam and X-rays to be covered, you must include their codes and fees in the HC Form. * You must list all treatment codes, sites and associated cost in the HC Form. It is most helpful to list as follows: * 23 33111 one canal root canal $xxx.xx * 23 27201 ACC crown $xxx.xx * When calculating the Total Cost requested for the HCFUND, include all costs for the listed treatment. If the patient has CDCP coverage, calculate the co-pay amount. * Only treatment included under the CDCP Fee Guide that meets CDCP eligibility criteria and frequency limits is considered. 3. There is a new Screening Template Note to be used in axiUm going forward. Located under D3X5 (3rd year) and D4X5 (4th year). 4. Radiology Request Form - you must close the requisition for radiographs REQ ---> FIN (page 71-73 in axiUm orientation manual) 5. Ensure you are checking in and checking out your patients when seated and dismissed- it is students responsibility to do so as they are in constant communication with their patient, as well as for safety reasons (mentioned in Patient Management Orientation). Part time instructors have mentioned this helps them monitor which students to check in with first. And this is mandatory in case of a fire drill all people must be accounted for and we use axium to track this. 6. Students are not allowed to delete treatment that is put in EHR. A patient's chart is a legal documentation - correct process is to submit correction card for it. (page 74 in axiUm orientation manual and page 36 in Patient Management Orientation) 7. Students are coming into PCCs windows with their masks under their chin. They touch their masks many times when by the window. Masks should be removed before exiting the unit followed by hand sanitizing. PCCs will be noting when students come to the front desk with masks on and breaches will be followed up. 8. Students must check SAP coverage for their patient prior to booking their patient's appointment.
