What to do if a patient cancels a planned re-evaluation
Good morning DH3,
I have received some clarification on what to do if a patient who is planned for a re-evaluation cancels that re-evaluation and declines to rebook. When this happens, please discuss with the patient on the phone what your recommended recall frequency would be and determine if that is agreeable to them or if they choose a different recall.
Then, please document in the EHR that the patient chose not to complete the re-evaluation so the DHCP goals were not evaluated. Indicate what your recall recommendation is and if the patient chose something different.
You can indicate somewhere in the DHCP (perhaps in the recommendations section at the bottom), that the evaluation wasn't completed as the patient canceled the appointment and declined to rebook.
The note, DHCP, and chart checklist will then need to be swiped by an instructor. The perio package code will also need to be completed and the patient's desired recall entered.
