OHAF Update
1. Approved OHAF funding * OHAF patient funding that has ALREADY been approved can proceed. * For procedure codes that are currently approved any minor changes such as 25 DO restoration changes to 25 MOD, or NPP3 becomes an NPP4 these changes can proceed without having to be reapproved. 2. Unapproved OHAF funding * Any treatment funding requests that have not already been approved WILL NOT be approved. * If the treatment you are asking to be funded is required to meet a course requirement, please send me an email explaining your situation. I will review your request with the course coordinator and the PCCs to determine what options are available. 3. Lab codes need to be swiped to Complete on the date of delivery. * If you accidentally have the Lab code swiped Complete ahead of the delivery, submit a Correction Card ASAP. The code will be set back to In process and available for you to swipe as Complete at the delivery appointment. 4. Reception/PCCs do not make any edits to patient charts. This must be done by the provider ahead of the delivery appointment. 5. D4 Hygiene Rotation – Recall Exam Codes are not being swiped during the D4 Recall rotation. Ensure that you are completing all forms including the recall or CTP codes. * You can leave the treatment plan swiped by the instructor and not signed by the patient. * When the patient is assigned to a dental student, that new dental student must review and confirm they agree with the treatment plan and at then either have the patient accept and sign the original treatment plan or create a new treatment plan if it needs to be modified.
1. Swipes before checking out at reception – all dental and dental hygiene students must enter a treatment code and treatment notes for each clinical session. You can in-process treatment codes multiple times. Please let your instructors know that they must swipe the procedure code BEFORE you dismiss your patient. 2. Send an email to request co-assignment/transfer between years to your course coordinator with PCC and classmate cc’d to email. Include details of the transfer. For D3 and D4 students, include Dr. Reeve in your email request. 3. Grade Card/Evaluation Forms – Confirm you are being graded for the proper Discipline and on the correct Grade Form to ensure accurate grades. If you notice an error, inform your grading instructor and ask them to redo on correct form. Submit a correction card asap.
