Hello Teacher Candidates,
The Practicum Office requires all Teacher Candidates currently in practicum to complete the mandatory Practicum Information Form linked below.
The form collects important information about your current placement, including your Mentor Teacher(s), teaching assignment and grade levels, opportunities within your teachable(s) and/or specialization areas, professional development, and the support you are receiving during practicum.

Completion of required practicum documentation is a professional obligation, and it is your responsibility to ensure the form is submitted by the deadline. Teacher Candidates who do not complete the form by the deadline will have a follow-up with the Practicum Director.
MTS Professional Development Day Clarification
As part of the form, you will be asked to provide information about your participation in professional development, which are a part of your practicum expectation.
Please remember that your MTS Professional Development Day activities must be equivalent to a full school day. This may consist of one full-day session or a combination of sessions that together are equivalent to a full school day.
Please provide information about the session(s) you attended when completing the form.
