As part of Canadian Patient Safety Week, the Canadian Patient Safety Institute has just released an updated version of the Canadian Disclosure Guidelines: Being Open with Patients and Families: http://www.patientsafetyinstitute.ca/english/toolsresources/disclosure/pages /default.aspx
This document, a revision of the disclosure guidelines originally published in 2008, aims to help organizations be open and honest with patients and families about safety incidents. CPSI's hope is that these guidelines will create new conversations about disclosure, clarify how best to meet the needs of patients and families, and help align disclosure policies across Canada. Learning organizations will take the opportunity to pause and reflect each time disclosure takes place, to learn from the experience, and make improvements in the system and in how they meet the needs of patients and families.
