I tried to complete the survey twice now but despite answer all questions it would not let me hit “Done” at the end of the survey because “This question requires an Answer” for Question 4 and 5 even though I have answered these questions and filled the boxes below each questions.

Dear Colleagues, The Office of Innovation and Scholarship in Medical Education (ISME) is conducting a needs assessment to better understand the opportunities, challenges, and support needs of faculty across the Max Rady College of Medicine. ISME is responsible for supporting educational programs across the College.
Regardless of your role in the College’s educational programs, whether teaching, research, curriculum, or leadership, your perspectives will help us identify priorities for future programming, resources, and services. We are particularly interested in understanding the educational challenges you anticipate over the coming years and the supports that would be most valuable in helping you succeed in your teaching, educational scholarship, and educational leadership roles. The survey will take approximately 5 minutes to complete. Responses will be reviewed in aggregate and will help inform future planning and resource development within ISME.
